Maine New Hire Package

If you ran out of forms, here is the required paperwork for new employees-

1. DHS and I require this within a few days of a new hire, make sure it is mailed or faxed ASAP.

 New Hire Report

2. Immigration requires you keep on file, make a copy of license and Social Security card.

 Form I-9 

3. Federal Income Tax Withholding, should be in before first paycheck, email the status/exemption number.

 Form W-4 

4. Maine Income Tax Withholding, store with federal, if numbers are not the same as federal, email me.

 Form W-4 ME 

Email if you have any questions and give my email address to employees, should they have a problem.
